Your local healthcare provider
Your current healthcare provider or a local doctor of your choice can also provide you with the necessary medical document. Book an appointment to find out if medical cannabis is right for you. Your provider can download our form to complete the documentation you’ll need.
Once they have filled out the medical document, create your Aurora Medical account and have your provider send us the form by fax to 1-403-637-3121. Feel free to browse our site for products, articles and more while you wait for your registration to be confirmed.
